Questionnaires
The questionnaire is a remarkably flexibly module that allows clients to create various types of questionnaires used for medical, safety, case, and industrial hygiene purposes. This module also has the flexibility of allowing users to add their own fields directly into the Medgate modules without the need to customize the system.
Any type of question can be entered including (Y/N, Numeric, Multiple Choice, Text, Date, checkbox, instruction, Normal/Abnormal or Table look up). Responses can be analyzed by either the source questionnaire or individual question, with options for data to be presented in graphic format.
There is no limit on the number of questionnaires that can be defined or the number of questions used to create questionnaires.
User-Defined Headers and Footers can be defined for each questionnaire, as well as the inclusion of company logos. Default answers can also be pre-defined.
