Incident Reporting and Investigation
This module captures information on incidents resulting in injury or illness and/or lost work time. For incidents resulting in lost time or medical care the relevant information will be transferred directly to the applicable Employers’ First Report of Injury for any state or province and to the OSHA 301 Form for US jurisdictions. Detailed statistical information is captured on each incident including the nature of the injury or illness, the body site affected, the source or cause of the incident, any underlying or contributing factors, and the activity the employee was performing at the time of the incident.
Root Cause Analysis:
Root cause analysis can be captured using the 5 Why methodology. Lessons learned can also be captured.
Corrective Action tracking:
Corrective Actions can be assigned automatically based on findings or can be added manually and assigned. Assigned e-mails can be sent directly through the system notifying the assignment of a corrective action. Reminders and notifications can be set-up to run automatically through Medgate’s Automatic e-mail notification system (MAEN). Users can define the frequency of reminders when the before the activity is due and once it becomes overdue.
A complete chain of custody for the investigation can be triggered through Medgate. E-mails, with links back to the system can be sent to the relevant managers based on demographic hierarchy i.e. manager to supervisor etc. to update the data live in the system until a final approval of the investigation is complete.
Reports:
Many statistical reports can be generated to see the patterns of incidents within your company. Other standard incident reports include the OSHA counts and OSHA rates reports as well as the OSHA 300 and 300A.
